Existing clients can request repeat scripts and referrals online, as well as request a change to an existing referral, request a medical certificate after a recent appointment. or chase pathology results. If your practitioner has asked you to complete a DASS or EDEQ form, you can find it here.

Phone: 1300 557 502. If you have a request that isn’t straightforward, we’d prefer an email. This allows time for our Reception team member to consult with others if required in order to provide the best possible solution. It also ensures the communication is more easily shared between staff if required.

Car park: There is time-limited parking on Elm Street, otherwise Northcote Plaza shopping centre is just a block away.
Fax: We are seeking to eliminate faxes. If you are a medical professional, we prefer Argus or HealthLink (ttcorner) and can exchange password-protected PDF files. In the absence of all other options, if you really, absolutely, have no other choice, then we can receive faxes on 03 9068 5249.
Transfer medical records: Health professionals can send large files to us using our upload facility.
After hours: We provide after-hours access for our patients via a mobile number that is shared when new clients join our practice. The after-hours number is not for medical emergencies. In that situation, please call 000 or attend your local Emergency Department.
